1) the tensioner failed/ was not set properly 2) the bearings on the timing pulleys or the studs which they are attached to failed and allowed the belt to lose tension (if you want peace of mind get these studs) 3) one or more of the camshafts jammed momentarily 4) the T-belt got soaked with oil or antifreeze and it became soft/stretched 5) if you run your timing belt without the covers, a rock or other road debris lodged itself between the gear and the belt and either stretched or jumped the T-belt
