It definitely sounds like the TBs are leaking, and everything you're ascertaining about the idle screw, AAC not able to lower idle, 12% indicating a vacuum leak, I agree 100% However I don't think your AAC is necessarily bad from what you've said here... did you try RAISING the idle with it? Like, set the AAC to 60% and see if the idle increases? It does bottom out around 12% for most cars, but as you know, being that low just indicates that there's more air entering than it can compensate for.
